We are presently accepting resumes for a permanent full-time Site Coordinator – Treatment Home to be responsible for the day-to-day operation and support matters related to the Empower Simcoe Treatment Home and planning for individuals receiving services through consultation and collaboration with the Manager of Clinical and Treatment services. Implement treatment plans to meet the direct needs of individuals with complex care needs. This position works up to 37.5 hours per week with a rate of pay of $34.49 per hour

Minimum Requirements for this position include:

-Community College Diploma or Degree in Behavioural Sciences, Social Sciences, Human Services, Police Foundations, Developmental services or a closely related field. Preference is given to graduates of a Behavioural Science Technology degree or diploma program;
-More than four (4) years recent full-time work experience supporting hard to serve adults with complex care needs in community residential settings. Experience supporting people with behaviour management techniques is an asset;
-Experience supporting teams with the implementation of clinical and or treatment plans in collaboration with teams, community partners and families;
-Extensive experience in supporting persons through applied behaviour management techniques are a critical part of this role. Registered Behavioural Technician certificate an asset;
-Demonstrates high level of skill and ability in Core Competencies: Advocating for Others; Collaboration, Creative Problem Solving & Decision Making; Fostering Independence in Others; Initiative; Interpersonal Relations & Respect; Resilience.

Duties and Responsibilities (including, but not limited to):

-Monitor the day-to-day caseload management of Treatment Home, by supporting the Treatment Home Support Workers in their day-to-day work in consultation with the Manager of Clinical and Treatment Services;
-Provide daily program updates to the Manager if they are working off site;
-Support the day-to-day shift coverage of staff related to program needs in consultation with the Manager of Clinical and Treatment Services and the Scheduling department;
-Provide direction to staff, as requested by the Manager, by assigning and prioritizing work to be completed and assisting staff with the completion of their tasks or assignments;
-Ensure people receiving services, employees, students, and volunteers receive a thorough orientation and ongoing support for growth and development;
-Working with the team, determine, schedule, attend and follow up on all medical appointments for each person;
-Participate in goal planning for supported individuals in accordance with the desire of the individuals and in conjunction with the Manager;
-Be responsible to have nutritious food and daily supplies available when needed; ordered and inventoried as appropriate;
-Share with the Manager assessment/ identification of functions of behaviour for each person in order to inform discussions at any multidisciplinary team meetings (internal and external members);
-Through ongoing training, as identified/supported by the Manager, lead others effectively in crisis situations through various approved verbal de-escalation and physical techniques;
-Work with the team to identify and complete all rights restriction for each person, for review by the Independent Rights Committee;
-Collaborate with the Quality and Innovation department to arrange Personal Outcome interviews within the first year of support for each person, and every three years after;
-Coordinate meetings, agenda, and follow up resulting from any internal meetings (team meetings) or external meetings (multidisciplinary external partnership model);
-To work in collaboration with the Manager and Treatment Home Support Workers to analyse Personal Outcome data, and effect changes to supports as needed or requested by each person supported;
-Conduct annual service satisfaction meetings with individuals supported;
-Foster the independence of those supported in the program and actively participate in transition planning for people supported and their families while maintaining accountability of the supports provided to each person;
-Monitor Passport spending to ensure program employees are supporting individuals to utilize funds in a timely and meaningful manner;
-Develop and maintain check lists/monitoring processes to ensure that tasks related to; all treatment plans, data collection/reporting, MCCSS/Accreditation’s standards, health, and safety standards are completed and are accurate;
-Ensure accurate and timely population of data into AIMS database by the Treatment Home Support Workers;
-Prepare for and participate in compliance related to Quality Assurance Measures MCCSS yearly compliance; and Accreditation Reviews;
-Oversee the ordering, storing and administration of medication to ensure all required medications are onsite, as necessary, in accordance with the Agency’s policies and procedures.

Special factors of this position:

-Engage in shift work, including evenings, nights, and weekend, as deemed necessary by the Manager; Participate fully in the afterhours on call process specific to the treatment home as determined/arranged by the Manager;
-Demonstrate the ability to respond effectively and calmly in crisis situations through various approved verbal de-escalation and physical intervention techniques. Position requires considerable physical interventions involving frequent bending, turning, twisting, and kneeling in response to crisis behaviour;
-Proficient in the administration of first aid, CPR, NVCI, and medication dispensing;
-Valid class G driver’s license and own reliable transportation to be used in the course of employment.
